Tennessee’s verified asbestos filing document is Department of Environment and Conservation form CN-1055, Notification of Demolition and/or Asbestos Renovation, revised October 2024. The form says to submit it 10 working days before the activity by email to Asbestos.NESHAP.Program@tn.gov. It implements 40 CFR 61.145 and Tennessee Division of Air Pollution Control Regulation 0400-30-38-.01(2)(b)(13). The form asks whether asbestos is present and, if it is, for a copy of the inspection report. Questions on the form go to (615) 969-5162. The form covers original, revised, courtesy, annual, and cancelled notices, and it distinguishes demolition, renovation, ordered demolition, and emergency renovation. Facility information includes the owner, the asbestos removal contractor, and any other operator. Scheduled dates cover preparation, asbestos removal, and demolition or renovation. The instructions say failure to tell the division about a start-date change before the activity can bring enforcement. The person who signs certifies that someone trained under 40 CFR Part 61 Subpart M will be on site during stripping and removal and that the training proof will be available.
Emergency renovations need the date and hour, a description of the sudden event, and an explanation of the unsafe condition, equipment damage, or financial burden. Ordered demolitions need the government order. Unexpected regulated material and nonfriable material that might be made friable must be described. Those fields are how Tennessee collects the federal notice content. They are not a license to start early. Courtesy notices are a box on the form, but the header still says to submit 10 working days prior to the activity.

Because Tennessee is not on the AHERA waiver list, schools follow the federal school rule in addition to this air notice when a school is renovated or demolished. OSHA’s construction standard applies to the crew.
